What Is a Perinatal Nurse Navigator (PNN)?

Envision PNNs are designated teammates who provide carefully guided support for higher risk patients. Our PNNs engage in a trusting relationship with their patients and care teams to ensure consistent communication, access to resources and seamless navigation throughout their pregnancy journey.

How We Work With You

• Provide emotional support and overall coordination of care
• Ensure ongoing and consistent communication
• Connect you with hospital and community resources
• Coordinate meet-and-greet appointments and facility tours with the clinical team
• Support your post discharge care and follow up
